Hello Carriers!

*Please read this entire Carriers section in full as this applies to ALL Carriers (CDL & Non-CDL)

Regional and OTR (Over The Road) Drivers only!

(Non-CDL) Box Truck requirements 22'ft and above!

JDL does NOT sign on Box Truck Carriers with 10ft/12ft/16ft equipment or Sprinter Vans!

JDL does NOT provide fuel cards or assist with fuel.

ALL Carriers MUST have:

*Factoring Company information (if using one)

*Copy of Valid Drivers License

*Active Authority

*MC#/DOT #

*COI (Certificate of Insurance) Cargo must be $100,000 Collision $1,000,000

*Completed W-9 Form (Completed & Signed)

*2 Pictures of your vehicle/equipment

*Blank Check (Voided) or Direct Deposit form for payout

All Carriers are required to have working equipment:

Lift Gate (if applicable), Pallet Jack, Straps, E-Track, Bars, Tarps, Door(s) and all other equipment.

JDL will NOT book any loads for a Carrier until the Carrier equipment is repaired and/or replaced.

If you meet all the requirements and have all required documents, go to the Carrier Questionnaire section of the website, complete the Questionnaire and submit to bookyourloads@jazzydlogisticsllc.com for review.

Once your Questionnaire has been reviewed and approved for sign on a

JDL Carrier Packet will be emailed to you for you to complete.

Upon completing the JDL Carrier Packet you will then have 3 days to email all required documents listed above to bookyourloads@jazzydlogisticsllc.com

Not supplying JDL with the required documents within a 3 day time frame of completing the Carrier Packet will be cause for your contract with JDL to be classified as void until all documents are received.

*Please note all forms are a REQUIRMENT and JDL will NOT be able to sign you on or book loads for you if all requirements are not met.

JDL Fees:

JDL charges a percentage of weekly gross pay which is 8% for all equipment types.
